The business:

As the largest not-for-profit health care system in Texas, Baylor Scott and White Health (BSWH) is “not only a provider of quality health care but also as a business that truly cares about people.” Working in the capacity of Service Design I helped BSWH to see a bigger goal: bringing preventive care to communities in need.

The challenge:

In 2023 BSWH Communities were seeing an above average CNI (Community Needs Index). Knowing that areas of need are least likely to attain preventive care and most likely to have barriers to accessing healthcare; Baylor needed to provide more patients with access to care.

The approach:
|
Focusing on Service Design, I needed to bring multiple business units together, focusing on a larger goal: bring BSWH Preventive Care Services to the communities it serves, instead of expecting communities ridden with barriers to come to them. Combining Preventive Care and In-Home Services we began a systemic approach to care, providing necessary preventive care vaccines and screening to the comfort of the home.
By working with a cross-functional team of Researchers, Developers, Business Architect, as well multiple operations teams, I architected a digital experience to allow patients to report or schedule multiple preventive care services, without ever having to set foot in an office. |
The Results:

Supported more than 4,500 patients to close Healthcare gaps by combining At-Home Services with Preventive Care outreach.
